Ketamine: an Emerging Treatment in Clinical & Acute Care Setting

February 8, 2022by Gisoo Zarrabi, M.D.

Ketamine treatment represents one of the most significant advances in psychiatric therapeutics in recent decades and is increasingly being integrated into modern, evidence-informed psychiatric care. At Newport Coast Psychiatry, our approach emphasizes individualized treatment planning and the thoughtful use of emerging, research-supported interventions for patients with complex or treatment-resistant conditions. For individuals who have not responded adequately to conventional psychiatric medications or psychotherapy alone, ketamine may offer a meaningful alternative within a carefully structured and monitored treatment plan.

Unlike traditional psychiatric medications, which primarily target serotonin, norepinephrine, or dopamine systems and often require weeks to demonstrate clinical benefit, ketamine works through a novel neurobiological mechanism involving glutamatergic pathways in the brain. Specifically, ketamine influences NMDA and AMPA receptor systems, leading to downstream effects on synaptic plasticity, neural connectivity, and neurotrophic signaling. Research suggests that ketamine may promote synaptogenesis, increase levels of brain-derived neurotrophic factor (BDNF), and enhance neural circuit flexibility, all of which may contribute to improvements in mood regulation, cognition, and emotional processing. These effects represent a shift from traditional symptom-suppression models toward interventions that may help restore underlying neural functioning.

Ketamine treatment may be considered for individuals experiencing treatment-resistant depression, severe depressive episodes, suicidal ideation, generalized anxiety disorder, post-traumatic stress disorder, obsessive-compulsive symptoms, chronic pain syndromes, fibromyalgia, neuropathic pain, or refractory mood instability, and more recently bipolar depression. Not every patient is an appropriate candidate, and careful psychiatric and medical evaluation is essential prior to initiating treatment. A comprehensive assessment allows for thoughtful determination of whether ketamine is suitable and how it may be integrated into a broader treatment strategy.

One of the most clinically meaningful aspects of ketamine is the potential for rapid symptom improvement in some patients. In contrast to traditional psychiatric medications, which may take weeks to produce noticeable change, ketamine has been associated with measurable improvements in mood, energy, and reduction in suicidal thinking within hours to days in certain individuals.

Although not all patients respond and maintenance strategies may be required, ketamine can serve as a valuable intervention for those who have experienced prolonged or severe symptoms and limited response to other treatments.

At Newport Coast Psychiatry, ketamine treatment is not provided in isolation but rather integrated into a comprehensive psychiatric care plan that may include ongoing medication management, psychotherapy, and individualized monitoring. This integrated model supports more precise and tolerable treatment planning, improves the likelihood of effective medication response, and may reduce the need for multiple medication trials, which is a common challenge in contemporary psychiatric practice. By combining biological, psychological, and pharmacologic approaches, treatment can be tailored more effectively to each individual’s needs.

Modern psychiatry is increasingly moving toward personalized and precision-based care. Advances in neuroscience and psychopharmacology are reshaping how clinicians understand and treat psychiatric illness. Ketamine represents one example of how emerging therapies can be incorporated into a broader strategy aimed at improving outcomes and quality of life. Our practice remains attentive to ongoing research and evolving clinical evidence so that patients may benefit from treatments that are both scientifically grounded and thoughtfully applied.

Ketamine treatment requires careful screening, monitoring, and follow-up to ensure safety and effectiveness. Prior to initiating treatment, patients undergo a detailed psychiatric and medical evaluation to assess appropriateness, identify potential risks, and clarify treatment goals. When used within a structured clinical setting and integrated into an overall treatment plan, ketamine can offer a meaningful option for individuals seeking relief from difficult-to-treat symptoms.

Newport Coast Psychiatry embraces a forward-thinking approach to mental health care that incorporates both established and emerging treatments. Our goal is to provide individualized, evidence-based care that improves functioning, reduces symptom burden, and supports long-term recovery. For patients who have struggled with limited response to traditional therapies, ketamine may represent an important component of a carefully designed and personalized treatment plan.
